one thing you mention about having a child is that shift from fears about the child while it's waiting to be born, and then there is that divide into fears about the child now that you have to take care of it and keep it safe and fed and happy. Is there going to be something of a divide in the game that might show both aspects of this as the story progresses? If you were to do say, 4 chapters (or nightmares?) i'd imagine the first 2 really focusing on fears about the baby internally and the folklore/aswangs associated with that and then have a middle story beat (either a brief cut scene or something showing a stepping point from that) and then the last 2 nightmares are more associated with the fears of caring for and raising a infant/newborn. Do you need to rescue it? protect it from creatures as you navigate the nightmare? solve a few puzzles to be able to get swaddling clothes or food or medicine for it? there is also the trust issues, who is OK to hold/watch/care for the child? would you create npc's that you COULD choose to care for it and discover it is a gamble and one of them is an aswang in disguise?
this is a bit more task oriented, but if the overall atmosphere is there, the fear would remain. plus, working to survive and also carrying a baby (outside, not inside) means additional stress as you can't RUN without potentially waking the baby and having it cry.. alerting monsters to charge and steal the child and eat it in front of you...
yeah.. that would be really horrific and scary...
I imagine you're only going to examine pregnancy itself and not really delve into the next steps.. but I'd love to see something a bit more divided since being a parent and raising kids is, as you said, a whole different story once the lil sucker comes out to see the light of day.

Well, we are taking it one step at at time and haven't really committed to what the later levels are going to be like. There will be some things that touch on fears of actually caring for a child a bit, but at least presently, the plan is to mostly focus on the fears before hand.
Since I am expecting an addition to my family in late August, my new perspective might change the direction of the game a little.
